The service receiver requires the software on Management Information System (MIS), Financial Information System (FIS) from the service provider. The service provider agrees to assume the responsibilities of the assignment and both the parties hereby enter into an agreement as follows: Article-I: Responsibilities ............................................................... The service provider will be responsible to install and provide operating support to built-up system in full. The Service receiver will ensure necessary co-operation to the Service provider for installation and operating    the software and have to acquire the technical know how within three months after installation of the    software. Article-II: Scope of Work ............................................................... The service provider will complete the following tasks. The Service provider will install the system at the selected computer Training at users level to be given so that they can use Front-end application easily. Three field visits will be made for installation and operating support and training at users level to be given so    that they can use front-end application only. The service provider will supply users manual. Article-III: Public Utteranc ............................................................... The software which would be installed will be the exclusive property of CDF and may not be communicated or    transferred to any other agencies in any manner. Article-IV: Warranty ............................................................... One-year warranty on the application software which will be provided by CDF. Any technical fault found in the program will be solved free of cost during the warranty period. It will not be applicable in case of any mishandling and system crash (damage of HDD). After warranty period CDF will provide technical services on cost covering basis. Any enhancement of modification of the program after necessary customization and installation of the system is not the part of this warranty. The extra tees for the modification or enhancement will depend on the effort for the job required. Article- V: Cost of Software ............................................................... This includes cost of products, orientation with 3-field visit, technical support for customization and installation of the system. The price is negotiable depending on the cases. MIS package Tk.30,000 FIS package Tk.30,000 (Both the packages Tk. 50,000) For additional installation charge Tk.10,000 per machine. Article-VI : Mode of payment ............................................................... Payments will be made in the following manner: First Installment : 75% of total cost (during signing of the agreement). Last Installment : Rest of total cost (upon completion of the work). Article- VII: Customization of the software ............................................................... The customization charge @ Tk. 2000 per man day is required for any further modification of MIS or FIS of the receiver at the database level, But customization for producing any forms and reports will be done free of cost. This rate is only applicable for Bangladesh.
